Enrolling Student Checklist 

The following documents must be included with your student registration application in order to complete the enrollment process.

            ____Enrollment Application Form with all spaces filled may be completed on line by clicking below.  You may need to load the latest JAVA program and authorize it, if your computer does not have it installed ( http://www.java.com/getjava ).  If you are still having a problem viewing this Enrollment Application Form, request an enrollment package from  the Admissions Committee (admissions@balboa-academy,org) or Fax  Panama (507) 211-3319.

We continue to receive Enrollment Applications on a hit or miss basis.  If you have not received a confirmation e-mail from the B.A. Office  within three business days, please copy and paste the application to an e-mail and send it to admissions@balboa-academy,org

             ____Three years of grades (current report card and previous two year’s report card, if applicable). Secondary students (Grades 9-12) need to submit a transcript of all credits obtained and report cards for 6th, 7th and 8th grade, if required.  Note: Report cards from grades 7-12 need to be validated thru Foreign Relations and Ministry of Education in Panama.  This applies to all countries except US and Mexico.  Out of country students need to submit a letter from the teacher or principal stating the academic/social behavior of the student.

            ____Copy of birth certificate and picture page of passport

            ____Vaccination Record  or Certification of Immunization below completed by your family doctor.

     If your Child(ren) are good candidates for enrollment and if there is space available at the requested grade level, you will be contacted by e-mail or phone to schedule the evaluation.
